Clinical Pharmacist Full Time

Provides clinical pharmaceutical care by planning, managing, monitoring, advising and reviewing specialized programs.
Formulates treatment plans for inpatients based on their symptoms,
medical history, previous responses to medication and suitable
pharmaceutical approaches and presents plans when requested in multidisciplinary meetings for discussion and review.
Conducts clinical review of drugs and other medications for the addition/deletion from the drug formulary.
Interview and document admission and discharge reconciliation of medication and provide discharge counseling explaining indication,directions of use, expected side effects
Prevents, detects and resolves medication therapy related problems including but not limited to drugs without indications, non-prescribed conditions, incomplete immunization records, inappropriate dosage and route/methods of administration, therapeutic duplication and medication allergies.
Verifies inpatient medication orders to ensure their effective delivery and accuracy.
Liaises with physicians, nurses and other clinical caregivers to ensure the delivery of safe, effective and economical drug treatments.

Education:

BSc in Pharmacy and PharmD or MBA or Master of Health Management.
Completion of specialty qualification as per the recognized specialty certificate (DOH/DHA guidelines).

Experience:

Minimum of 2 years experience post qualification in related field in hospital setting.
